Work Permits

Work permits are distributed by the Academic School Counseling Department throughout the school year. Please see your counselor once you have followed the following directions. 

Once a student is offered employment, the student downloads the "Statement of Intent to Employ Minor and Request for Work Permit" form found below.

When the "Statement of Intent to Employ Minor and Request for Work Permit" with the following completed sections: Minor's Information, School Information, Employer Information to Complete (give to your employer to fill out), and Parent Signature, the student can see his/her academic school counselor to complete and return the paperwork on the spot.  

 ***Students may also obtain permits OVER the SUMMER from Mrs. Fitzpatrick in the Main Office.***
