Immunizations

In considering the health and safety of all students, faculty and staff, the State of California, the Archdiocese of Los Angeles and Bishop Montgomery HS require updated health and inoculation records are on file for each student. A student having incomplete health records may be suspended until the matter is cleared.

The Following inoculations are required for your student to attend school:

• Polio (OPV or IPV)-5 doses
• DTP/DTa/DT/TD (Diphtheria, tetanus, and [acellular] pertussis OR tetanus and diphtheria only)-6 doses
• MMR (Measles, mumps, and rubella)-2 doses
• Hepatitis B-3 dose
• Varicella-2 doses
• TB Skin Tests
